Fix older maier/alpine-3.3* boxes to update apk repository. (replace defunct repos.dfw.lax-noc.com with dl-cdn.alpinelinux.org when installing nfs client.
Corrections to readme and example URLs. Add shell provisioner to example to update OS on vagrant up.
