Improve comments on Pull Requests.

• Use Uffizzi's new predictable URL's before deployment starts.

Add reusable workflow inputs:

• URL path for post-deployment healthcheck
• Git reference, for use when called by workflow_run event

Add reusable workflow output:

• Uffizzi dashboard URL for containers just deployed

Remove deprecated set-output commands per GitHub https://github.blog/changelog/2022-10-11-github-actions-deprecating-save-state-and-set-output-commands/

Documentation updates to README

Use metadata labels instead of comments to track which deployments track with which Pull Requests. Comments are still posted and updated but are no longer used for decisions.

• Include example reusable workflow.
• Add support for new Uffizzi feature: deployment access control
