Bugfixes, support APEX versions from 5.1 to 20.2

1. Wrong aggregations or some ORA-errors if IR has hidden column used in links, substitutions like #COLUMN# etc.
2. Wrong aggregations or some ORA-errors if IR has non-displayed aggregation column(s).
3. Improved logging
4. Custom rendering engines are deprecated.
5. UI change - no icon on the "XLSX" - button for IG anymore. The function to show this button is completely changed, to support different versions of APEX, custom styling and Themes.
6. PL/SQL refactoring - thousand of comments, code was formatted, some variables and functions was renamed to the to improve understanding.
7. Tested for all supported APEX versions: 5.1.4, 18.1, 18.2, 19.1, 19.2
