gulp + nodemon + convenience
Once again in compatibility with nodemon's new change event message.
No major changes, just updates to README and a version bump for NPM.
Fixes two bugs and adds a useful 'change' event for running code before the server restarts.
Refactors the whole plugin, closes all but one bug, and provides fallback to the default nodemon stream.
